Hi Nickie,
The Premium version integrates 7 other plugins such as keywords, subscribe to comments, adds a Twitter field, GrowMap anti-spambot plugin, and provides incentives for your commenters to +1 your posts on Google Plus and share them on Twitter and Facebook.
The free version simply adds the CommentLuv field and none of these other functions.
The only thing I’m wondering about is the sententce:

Which plugins can be skipped exactly? I’m not sure which you mean here…

Yes, CommentLuv and CommentLuv Premium are both configurable dofollow OR nofollow – but if you see nofollow links in my comments let me know because they should not be. Either I allow a link or I remove it (if objectionable) but I don’t nofollow them.
commentluv premium is more for getting back links and increase traffic to your site. I have been able to grow more quality back links to my sites.

I think Comment Luv has been one of the most important aspects of my link-building strategy. Commentluv is valuable for so many reasons. While it encourages commenters, I think it actually REDUCES spam comments because the link needs to be referring to a blog with real content for it to be effective.
